One-Month HEPA Purifier Use Linked to Faster Executive-Function Test Scores

In a randomized crossover study of 119 Massachusetts residents, participants aged 40 and older completed an executive-function and mental-flexibility test about 12% faster after a month using a HEPA air purifier than after using a sham device. The study did not establish whether the effect lasts or applies to adults over 60.

Adults aged 40 and older completed a test of executive function about 12% faster after using a HEPA air purifier at home for one month, according to a secondary outcome analysis published in Scientific Reports. The finding came from a randomized crossover trial involving 119 people living in Somerville, Massachusetts, an area near major highways with relatively high traffic-related air pollution.

Participants were assigned to use either a functioning HEPA purifier or a sham purifier for one month, followed by a monthlong break and then one month with the other device. The sham purifier looked and operated like the real device but did not contain an air-cleaning filter. After each month, participants completed tests assessing visual memory, motor speed, executive function and mental flexibility.

The faster performance appeared in the study’s participants aged 40 and older, who represented about 42% of the sample. This group completed the test section requiring them to connect alternating numbers and letters—a measure of executive function and mental flexibility—12% faster after the HEPA-purifier period than after the sham-purifier period. The analysis accounted for factors including time spent indoors and how stressful participants found the test.

The report, covered by ScienceDaily using materials from The Conversation, describes HEPA filtration as a way to remove particulate matter from indoor air. The researchers connected the study to broader concerns about traffic-related pollution and cognitive function, but the trial measured performance on specific mental tasks rather than diagnosing neurological disease or demonstrating protection against conditions such as Alzheimer’s or Parkinson’s disease.

The age-specific result is important because the study’s overall sample ranged from 30 to 74, while the reported improvement was observed among participants aged 40 and older. Fewer than 10 participants were over 60, leaving limited information about the purifier’s effects in older adults. The study also tested only one month of air purification, so it did not determine whether the improvement would persist, grow or translate into reduced cognitive decline over a longer period.

The researchers said the biological explanation remains uncertain. They pointed to previous research suggesting that particulate exposure may affect the brain’s white matter, which helps nerve cells transmit electrical signals and maintain connections between brain regions. They plan to investigate whether reducing particulate matter changes white matter or alters levels of cellular metabolites, but those questions were not answered by this study.

The findings therefore suggest a short-term association between home HEPA filtration and performance on one measure of executive function in adults aged 40 and older. They do not establish a long-term cognitive benefit, a reversal of cognitive decline or a causal pathway linking cleaner indoor air to improved brain health.
